Voices of the Community: Insights from Survivors, Allies, and Advocates
The Sudden Cardiac Arrest Foundation is conducting a survey to gather perspectives from survivors, co-survivors, advocates, supporters, rescuers, trainers, medical professionals, partners, and others involved in sudden cardiac arrest awareness, prevention, education, and advocacy. If you have interacted with SCAF, utilized any of our services, resources, or educational materials, partnered with us or supported our efforts, we want to hear from YOU!
Your feedback will help us better understand the challenges you face, the support you need, and the impact of our current programs. Most importantly, your input will guide the development of new resources, outreach efforts, and effective awareness campaigns — ensuring that we continue to empower communities and save lives.
This survey should take less than 10 minutes to complete. Your responses will remain confidential and will be used only in aggregate and de-identified form to improve our programs and share insights and key learnings.
As a thank you for your time and feedback, you can enter a drawing to receive an exclusive Sudden Cardiac Arrest Foundation swag bag — including a T-shirt, hat, stress ball, and stickers!
Thank you for being part of this important effort!
